YouTube Kids keeps showing inappropriate stuff

Okay so I thought YouTube Kids was supposed to be filtered but my 5yo somehow got to a video about a dentist pulling teeth and it was graphic as hell. She was genuinely scared. I've restricted her to a specific playlist now but honestly the app is kind of a mess. Does anyone just use regular YouTube with parental controls instead? What's your setup?
