This is the third time in two months she's asking to swap weekends with no notice. I get that life happens but I plan stuff with the kids and it messes with them too. How do I handle this without being the bad guy? Do I just start saying no or is there a better way to address it?

Third time is a pattern, not a coincidence. You need to put your foot down about this. What does your custody agreement actually say about schedule changes?
Document each request and your responses. If this keeps happening, you'll have proof for your lawyer. Don't let this slide.
