Lawyer said I need documentation - how detailed does this need to be?

Going back to court next month to adjust custody. My lawyer wants me keeping detailed records of everything - pickups, missed calls, conversations, behavioral issues with the kids, everything. Feels kind of paranoid but I get it. Does anyone have a template or system they use? Don't want to look like I'm obsessively tracking stuff but need to have my facts straight.
